CVU Learning Academies reimburse the cost of certification exams up to two attempts. The reimbursement funding is provided through corporate funding vs. a program’s budget. This exam reimbursement is for any certification exam listed on the learning academy page.

Return to this form if the available training does not meet your need and provide a justification.

Before completing this form, make sure you know the following information, as these will be required fields:

• Billing Type/GL Account Code
• Org Code
• Project String

If you are unsure, consult with your manager to confirm how this training will be funded prior to filling out the form.
